School Based Health Center
SCHOOL CLOSURE INFO DUE TO COVID-19 PRECAUTIONS:
The School-Based Health Center is a unique venture between the Utica City School District and Upstate Family Health Center, Inc. With no out of pocket expense to you, the School-Based Health Center provides comprehensive primary health care services exclusively to enrolled students in participating schools. Staffing includes a full time provider, a supervising physician and a full time licensed practical nurse.
Children must be students at MLK Elementary School and parents must complete an enrollment packet in order to receive healthcare services. Please contact the SBHC staff at (315) 368-6730 for an appointment and enrollment information.
****For students that have previously used the SBHC services, an enrollment packet will be sent in the mail to the parent/guardian in August. Please fill out completely and return it in the envelope provided.****
We offer weekday appointments before, during and after school during the school year. We are closed when school is not in session, including holidays and summer break.
When the health center is closed, students and their families are encouraged to call our main office for medical services.
